Camp CREATE Financial Aid Application
In order for us to assess the level of support we are able provide, please fill out the following information and upload a copy of one of the following: (1) Most recent IRS 1040 income tax return, (2) Current eligibility/award letter for SNAP, TANF or Oregon Health Plan or (3) Other documentation of income (SSA/SSI Benefit Statements, or last 3 months of paycheck stubs, etc.). All application materials will be kept confidential.
It is ideal for this application to be submitted before February 24th, in time for registration opening on February 26th. If it is after February 24th, please email or call us and we will work with you to assure an equitable process.
Once your application is submitted, please plan to wait 2-5 days to hear back from us with a letter of financial support along with a code and registration instructions
If you are a Cascades Academy family that receives financial assistance, no IRS 1040 income tax return is needed with your application.
